Timesheet Sources and Employee Mileage Rates

Timesheet responses now include in_source and out_source, showing how each punch was
recorded. Applies to list, get, create, update, clock-in and clock-out.
User responses now include mileage_rate, the employee's per-mile reimbursement rate.

mileage_rate can be set when creating or updating a user. Send null to clear it so the
company rate applies again.mileage_rate returns the company rate when an employee has no rate of their own, so the field is always populated.
